    Objetiva-se, por intermédio da presente pesquisa científica, tratar acerca das dimensões existentes no âmbito do princípio da presunção de inocência, bem como avaliar as implicações jurídicas ali decorrentes. Nesse prumo, tem-se que o princípio da presunção de inocência se encontra previsto no cerne da Magna Carta de 1988 e se traduz como um dos pilares do Estado Democrático de Direito. Apesar de se tratar de um princípio de inestimável importância, haja vista que não apenas garante a proteção contra arbitrariedades estatais, mas também regula as relações processuais penais, o mesmo tem sofrido interpretações divergentes por parte do Superior Tribunal de Justiça. Esse fato resulta em insegurança jurídica, uma vez que alterações frequentes na jurisprudência, impactam diretamente o equilíbrio entre os direitos fundamentais dos acusados e a efetividade da justiça penal. Com subsídio nesses pressupostos de análise, coaduna-se com o posicionamento de que a relativização da presunção de inocência traz impactos significativos, comprometendo diversas garantias constitucionais e demais princípios basilares do processo penal, além de trazer implicações concretas à sociedade. Portanto, mister que os estudos futuros explorem a interação entre a presunção de inocência e outros princípios constitucionais, como o princípio da proporcionalidade, buscando um ponto de equilíbrio que assegure tanto os direitos fundamentais dos indivíduos quanto a eficiência da justiça criminal. Com relação à metodologia adotada no presente artigo científico, enfatiza-se a utilização de instrumentos bibliográficos, doutrinários e jurisprudenciais, haja vista que a temática em comento é predominantemente teórica e com critérios qualitativos.

  • IBC Oxford University Oral Abstract 33 - The role of bariatric surgery in the management of psoriasis: a systematic review and meta-analysis

    Abstract Background Psoriasis is a chronic immune-mediated skin disease that is more prevalent and severe in obese patients. To the best of our knowledge, no systematic review about the influence of bariatric surgery in the management of psoriasis has been conducted until now. Therefore, we aimed to systematically review all the current literature about the presumptive positive effects of bariatric surgery in the management of psoriasis, evaluating the impact of surgery in psoriasis improvement. Methods We conducted our systematic review and meta-analysis in accordance with the guidelines outlined in the Preferred Reporting Items for Systematic Reviews and Meta- Analyses (PRISMA). We searched three major databases: PubMed, Scopus, and Web of Science. We evaluated qualitatively the included studies and in our meta-analysis, we focused on evaluating the dichotomous variable of ‘Downgrade of Systemic Medication after Surgery,’ an indirect measure of psoriasis improvement, analyzing the proportion of patients experiencing this outcome. Results The analysis included four studies with a total of 127 participants and 69.3% of them were women. Roux-en-Y Gastric Bypass was the most common procedure (78%). Three studies evaluated psoriasis improvement post-surgery, showing improvements ranging from 39.4% to 70% based on DLQI score, ABSA score, and even on subjective evaluation by the patients. Metabolic comorbidities improvement was confirmed in two studies and older age was also associated with a higher probability of improvement. Results from the meta-analysis showed that out of 67 patients using systemic medications, 45% (Proportion = 0.45, 95% c.i. [0.33;0.57]) experienced reduced medication post-surgery. Conclusion Although there is promising evidence of psoriasis improvement following bariatric surgery, the limited number of studies and participants prevents broad generalization to real-world situations. This underscores the necessity for additional research, particularly randomized studies, to determine whether bariatric surgery offers superior outcomes compared to alternative weight loss interventions for managing and improving psoriasis in obese patients.

    Background Pulmonary atelectasis is a commonly occurs during anesthesia. In these cases, mechanical ventilation (MV) associated with alveolar recruitment maneuvers (ARMs) and positive end-expiratory pressure (PEEP) is indicated to reverse the condition, ensure adequate gas exchange and improve oxygenation. ARMs can trigger volutrauma, barotrauma, and atelectrauma. Therefore, computed tomography (CT) is the gold-standard method for monitoring lung aeration after ARM. Objective To evaluate lung volume distribution after stepwise ARMs using computed tomography (CT). Methods Twelve dogs weighing 24.0 ± 6.0 kg, aged 3 ± 1 years, of both sexes and different breeds, underwent orchiectomy or ovariohysterectomy. The animals were anesthetized and ventilated in volume-controlled mode. ARMs were then initiated by positive end-expiratory pressure (PEEP) titration (5, 10, 15, and 20 cmH 2 O). CT scans, cardiovascular parameters, and ventilatory mechanics were evaluated at all time points. Data were assessed for normality using the Shapiro–Wilk test and a two-way analysis of variance, followed by a post-hoc Bonferroni test to identify differences between time points. Statistical significance was attributed to a value of p of <0.05. Results CT demonstrated that the ARMs increased ventilation throughout the lung, including the dependent regions, with volumes that increased and decreased proportionally with PEEP titration. When they reached PEEP 10 and 5 cmH 2 O descending (d), they remained significantly higher than those in PEEP 0 cmH 2 O (baseline). Static compliance improved about 40% at PEEP 10d and PEEP 5d compared to baseline. There was an increase in heart rate (HR) from PEEP 15 increasing (i) (74.5%) to PEEP 10d (54.8%) compared to baseline. Mean arterial blood pressure (MABP) decreased approximately 9% from PEEP 15i to PEEP 15d compared to baseline. Conclusion Lung attenuation and regional and global volumes assessed by CT showed that maximum pulmonary aeration distribution followed by PEEP titration occurred at PEEP 20 cmH 2 O, maintaining the lungs normoaerated and without hyperaeration.

  • Unilateral Vocal Fold Paralysis and Voice Therapy: Predictors of Long-Term Quality of Life

    To date, little is known about the long-term predictors of quality of life (QoL) in unilateral vocal fold paralysis (UVFP). The main objective of this study was to evaluate the predictors of long-term QoL in UVFP patients submitted to voice therapy (VT) exclusively. Data from patients diagnosed with UVFP who followed a VT program between 2013 and 2019 were reviewed. Video laryngoscopy (VL) records were obtained at the beginning and at the end of VT. To assess QoL, Voice Handicap Index 30 (VHI-30) score was assessed in three temporal frames: before voice therapy (pre-VT), at the last VT session (post-VT), and in the present (cur-VHI). A longitudinal analysis was performed regarding the evolution of QoL and the factors influencing QoL through time were analyzed. Seventy-eight percent of patients had iatrogenic UVFP. The mean time of follow-up after VT was 3.942 years (range 6 months-7 years). There was a significant improvement in QoL through all time points (F (2,88)=72.179, p<0.001), with VHI-30 decrease from the baseline pre-VT to post-VT(p<0.001) and from post-VT to cur-VT (p=0.0013). In the iatrogenic UVFP population, patients starting VT earlier showed better long-term QoL (p=0.023). UVFP patients with dysphagia at presentation showed significantly worse QoL in the late follow-up (p=0.016). Hence, iatrogenic UVFP patients beginning VT rapidly may show better QoL in the future. Also, our results suggest that dysphagia at UVFP onset may predict higher morbidity later in life.
