    Introduction: Eat, Sleep, Console (ESC) is an effective approach for evaluating and managing neonatal opioid withdrawal syndrome (NOWS). The current standard, Finnegan Neonatal Abstinence Scoring System, requires waking neonates to assess NOWS and prioritizes pharmacotherapy treatment. However, ESC focuses on infants' abilities to function and cope with opioid withdrawal, prioritizes nonpharmacologic interventions, and emphasizes the crucial role of the parent-infant relationship. We created and delivered ESC training for perinatal and neonatal staff and clinicians across an urban academic health center. Methods: We utilized the knowledge-to-action framework to guide project design and implementation. The training program consisted of 30- to 60-minute didactic sessions for neonatal and perinatal clinicians and staff on labor and delivery and neonatal intensive care units, an ESC algorithm for care, and pre- and posttraining surveys. Results: = .03;). Pre/posttraining unpaired survey results indicated high levels of preparedness implementing ESC concepts. Discussion: ESC education enhanced preparedness of birthing staff and clinicians to implement the nonpharmacologic ESC tool for management of NOWS. Coordinated, multidisciplinary education and collaboration support the successful implementation of ESC in clinical settings.

  • Faricimab for previously-treated eyes with neovascular age-related macular degeneration with inadequate response – 6-month interim analysis of the FURGGHORN study

    PURPOSE: To investigate the effect of switching to faricimab therapy patients with neovascular age-related macular degeneration currently treated with anti-VEGF therapy. DESIGN: Phase IV, prospective, single-arm, open-label, multi-center, investigator-initiated clinical trial. METHODS: Eligible eyes received four initial doses of faricimab 6 mg/0.05 mL every 4-weeks. From week 12, dosing intervals were increased to eight weeks and then modified according to a treat-and-extend based regimen. This interim analysis reports changes in best-corrected visual acuity (BCVA), central subfield thickness (CST), retinal fluid proportions, and the incidence of adverse events from baseline through to week 24. RESULTS: One-hundred-and-two patients of mean age 76.0 years were recruited from 12 sites across Australia and Singapore. Baseline BCVA was 72 Early Treatment Diabetic Retinopathy Study letters and baseline CST was 384 µm. Seventy-two percent of eyes were being treated with aflibercept 2 mg and 28% with ranibizumab. After switching to faricimab, mean BCVA change was + 1.9 letters at week 12 (P < 0.001), and + 1.9 letters at week 24 (P < 0.001). Mean CST change was -83.9 µm at week 12 (P < 0.001), and -82.2 µm at week 24 (P < 0.001). The proportion of eyes with fluid resolution was 54% at week 12, and 59% at week 24. The proportion of eyes assigned to 4-weekly, 8-weekly, and 12-weekly treatment intervals were 16%, 35% and 49% respectively at week 24. Anterior chamber inflammation occurred in one eye with no cases of vasculitis or occlusive retinitis. CONCLUSIONS: In this pre-specified interim analysis of a prospective study, patients switching to faricimab experienced improved anatomic outcomes and maintained visual function, with a large proportion assigned extended dosing intervals under a flexible treatment regimen.     Postpartum patients experience a 60-fold increased risk for venous thromboembolism (VTE). We present a postpartum patient with severe pre-eclampsia, gestational diabetes, and a recent Cesarean delivery, who was diagnosed with a VTE hours after a Nexplanon insertion on venous duplex ultrasound. She was started on anticoagulation, had the Nexplanon removed, and recovered well. This case highlights the importance of clinical suspicion for VTE in the postpartum period, presenting a postpartum VTE coinciding with a subdermal implant placement.

    In 2011 is er een grootschalige strandsuppletie (Zandmotor) van 21.5 miljoen m3 uitgevoerd aan de Hollandse kust tussen Ter Heijde en Kijkduin. De Zandmotor wordt gezien als een mogelijk alternatief voor de kleinere, reguliere strand- en vooroeversuppleties. Het doel van dit experiment is na te gaan of het eenmalig aanbrengen van een grote hoeveelheid zand uiteindelijk goedkoper, effectiever en natuurvriendelijker is dan het herhaaldelijk aanbrengen van kleinere hoeveelheden zoals gebeurt bij de reguliere vooroever- en strandsuppleties. Om dit in kaart te brengen is in 2010 een grootschalig monitoringsprogramma opgezet. Een van de onderwerpen binnen het monitoringsprogramma is het volgen van de ontwikkeling van de bodemdiergemeenschap in de vooroever en het natte strand. Hiervoor zijn in het najaar van 2010, 2012, 2013, 2015, 2017, 2019 en 2024 sediment en bodemdiermonsters verzameld en geanalyseerd.. Van 16 september tot en met 24 oktober 2024 zijn er bemonsteringen uitgevoerd van de bodemdiergemeenschap en sedimentkarakteristieken op en rond de Zandmotor. In totaal zijn er 120 stations in de vooroever bemonsterd met een Van Veen-happer en een bodemschaaf. Tevens zijn er 70 stations bemonsterd op het strand tussen de hoog- en laagwaterlijn. De bemonsterde stations liggen op 12 parallelle transecten loodrecht op de voormalige kustlijn met een onderlinge afstand van 800 tot 1000 meter. In voorliggende rapportage worden de resultaten van de resultaten van de analyses op sedimentsamenstelling (korrelgrootteverdeling en organisch stofgehalte) en bodemdiergemeenschap gepresenteerd en vergeleken met voorgaande jaren (2010, 2012, 2013, 2015, 2017 en 2019). Er zijn duidelijke ruimtelijke patronen in sedimentsamenstelling met gebieden waar het sediment relatief grof is (kop van de Zandmotor en het meest noordelijk deel van onderzoeksgebied) en gebieden waar het sediment fijner is (net ten noorden en ten zuiden van de Zandmotor en in het meest zuidelijke deel van het onderzoeksgebied). Deels zijn deze patronen het directe gevolg van de Zandmotor. De gemiddelde biomassa aan bodemdieren (met weglating van Ensis spp., strandgapers, otterschelpen en heremietkreeften) in de bodemschaaf in 2024 (48 gram versgewicht m-2) is minder dan in 2017 en 2019 (respectievelijk 85 en 75 g m-2) maar aanzienlijk hoger dan in de jaren 2010, 2012, 2013 en 2015 (respectievelijk 1, 23 en 14 gram versgewicht m-2). Het gemiddeld aantal taxa per monster was 5.6. Dit is minder dan in voorgaande jaren. Ensis spp., halfgeknotte strandschelp (Spisula subtruncata), nonnetje (Macoma balthica) en de gewone slangster (Ophiura ophiura) waren de meest dominante soorten in de bodemschaaf met gemiddelde dichtheden van respectievelijk 44, 23, 4 en 3 individuen m-2. Opvallend is dat de Amerikaanse strandschelp (Mulinia lateralis), een nieuwkomer die in 2019 voor het eerst is aangetroffen bij de Zandmotor (20% van de bemonsterde locaties), in 2024 niet meer is aangetroffen. In de Van Veen bemonstering zijn gemiddeld meer taxa aangetroffen (in 2024, 13.1 taxa per monster) dan in de bodemschaaf. Dit komt door de fijnere maaswijdte van de zeef bij de Van Veen bemonstering (1 mm) ten opzichte van de bodemschaaf (5 mm). In het najaar van 2024 kwamen de soorten als Nephtys cirrosa, Spiophanes bombyx, Spio spp., zeekomma (Diastylis bradyi), Owenia spp., de aasgarnaal Schistomysis kervillei, het vlokreeftje (Urothoe poseidonis), de steeloogaasgarnaal (Mesopodopsis slabberi) en de gewone slangster (Ophiura ophiura) relatief vaak voor. Het totale bestand aan bodemdieren (dichtheid, biomassa en aantal soorten) neemt toe met de waterdiepte. Vooral de ondiepe zone (tot en met NAP -5 meter) is arm en wordt gekenmerkt door specifieke soorten zoals de breedpootkrab (Portumnus latipes) en de gemshorenworm (Scolelepis (Scolelepis) squamata). Op grotere diepten (dieper dan NAP -6 m) neemt de dichtheid, biomassa en het aantal soorten per monster sterk toe, met de hoogste waarden op dieptes meer dan NAP -11m. De aanleg van de Zandmotor heeft vrijwel direct geleid tot een grotere variatie in habitats, waardoor de diversiteit van de bodemdiergemeenschap is toegenomen. Dertien jaar na aanleg is de bodemdiergemeenschap nog niet hersteld naar de oorspronkelijke situatie. Een toename van langlevende soorten, zoals vooraf was verwacht, is nog niet duidelijk zichtbaar. Waarschijnlijk komt dit door de dynamiek in de ondiepe vooroever, waar bodemdieren voortdurend te maken hebben met verstoring en herstel. Het herstel van de bodemdiergemeenschap kan nog vele jaren duren, omdat de Zandmotor komende jaren nog niet zal verdwijnen en door zijn aanwezigheid de stroming, sedimentatie en erosie in het gebied blijft beïnvloeden.

  • Key stakeholders’ experiences, knowledge and perspectives regarding care quality for breast cancer in South-West Nigeria

    The landscape of breast cancer care in Nigeria is complex, with various structural and individual barriers impacting patient care. Breast cancer (BC) is the most common cancer and a leading cause of cancer deaths among women worldwide. In Africa, the cancer burden is expected to rise significantly, with projections estimating an increase of 50% by 2050. Rising incidence rates and barriers to care contribute to a healthcare crisis, leading to late-stage presentation and high mortality rates for women with breast cancer in Nigeria. Quality healthcare must be patient-centered, involving stakeholders - patients, clinical and community partners, and other healthcare stakeholders to achieve a desired outcome. Understanding the cancer journey from different perspectives allows for targeted approaches for increasing access to quality healthcare as well as reducing morbidity and mortality rates. To address this, healthcare provider perspectives about breast cancer care were compared with the lived experiences of breast cancer patients to emphasize the need to increase access and quality of care. A mixed method study was conducted in 2 phases: Phase I: 3 Focus group discussions (FGDs) with breast cancer patients and their care givers were conducted at the NSIA-LUTH Cancer Centre in Lagos, Nigeria. Phase II: A pre and post-survey of a continuing medical education course focused on breast cancer was delivered to healthcare providers in southwest Nigeria. Survey responses regarding causes for delays and barriers to care indicated financial strain, fear, and alternative treatments as the largest hurdles, coinciding with patient testimony from the FGD. Fear of mastectomy was a perceived barrier to care for 90% of healthcare providers while 87% and 86% of providers perceived seeking spiritual and herbal treatments as the largest delays of care. Despite this, a significant number of focus group participants (39%) presented within the first month of noticing a breast symptom to a proper healthcare provider. Data from our study reports that 70% of patients receive help from family to fund treatment highlighting why cancer can be a poverty trap for families and the need for universal health insurance. Half of the focus group participants had a positive interaction with their doctors, with the rest reporting neutral (19%) or even negative (31%) interactions. Our study also reports 42% of healthcare providers feeling only "somewhat" qualified to deal with breast cancer, highlighting the significant need for more education, with a further 14% feeling neutral or negative about their qualification, a potential contributing factor in negative interactions recalled by patients. Knowledge increase was consistent for best practice diagnostic modalities among healthcare providers (p < 0.05). At the same time, items related to symptoms and risks of breast cancer had inconsistent knowledge increases, indicating why further courses like these should be pursued. With the success of the course and the inspiration of breast cancer survivors, a proposed expansion into community awareness is discussed along with enlisting local practitioners in the fight against breast cancer in hopes of lowering the barriers to and delays of care in Nigeria.
